Evaluating the Impact of Robotic Surgery Experience on the Outcomes of ORP

The following is a summary of “Effects of robotic surgery experience on open radical prostatectomy results: single centre & single surgeon experience,” published in the December 2024 issue of Urology by Gul et al. 


Researchers conducted a retrospective study to evaluate the impact of robotic surgery experience on open radical prostatectomy (ORP) outcomes, comparing results before and after the surgeon gained robotic surgery experience. 

They retrospectively reviewed 100 patients who underwent ORP for clinically localized prostate cancer (PCa). Group 1 consisted of the last 50 patients before the robotic surgery experience, and Group 2 included the first 50 patients after the robotic surgery experience. Demographics, preoperative data, and perioperative outcomes were recorded, with continence and erectile functions evaluated preoperatively and at 1, 3, 6, and 12 months postoperatively. 

The results showed similar demographics and preoperative data between the groups. Group 2 had higher preoperative and postoperative Gleason scores (P = 0.001, 0.033). Significant differences included operation time (200 vs 115 min), blood loss (400 vs 200 ml), transfusion rate (46% vs 8%), drain removal time (4 vs 3 days), hospital stay (5 vs 4 days), and catheter removal time (21 vs 14 days) (P < 0.001). Group 2 had a higher continence rate at 1 month (P = 0.027), with no significant difference at 3, 6, or 12 months. Erectile function showed no significant difference (mean IIEF: 9.47 vs. 11.18 at 12 months, P = 0.220). 

They concluded that robotic surgery improved outcomes, offering better visualization, reduced bleeding and transfusion needs, shorter operation time, hospital stay, catheter removal, and enhanced early continence.
